29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:   On or about February 14, 2013, employees were working on Rig #553. Two energized flexible cords were run through the floor of the mud pit, one of the cords had damaged insulation. This condition exposed employees to potential electrical hazards.
