29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about February 14, 2013, Rig 150 drill site, The Mud loggers trailer grounding electrode conductor attached to the ground rod (electrode) was not of the correct size for effective grounding. Employees were exposed to electrical shock and/or electrocution.
