29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) Construction Site: On or about March 08, 2013, and at times prior thereto, employees were exposed to an ejection and fall of approximately thirty feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
