29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Warehouse - Employees used a "Task Force" 12 inch compound miter saw serial number 019764, to cut wood stock to manufacture pallets and shipping containers. The guard intended to protect the employees from the rotating saw blade did not return to the lowered position when the operator was finished with a cut, the operator was exposed to the rotating 12 inch saw blade on or about March 12, 2013.

The tool rest was more than 1/8 inch from the face of the abrasive grinding wheel and the bolt securing the tool rest at a specific angle was not tightened. Employees were exposed to the danger of parts getting pulled from their hands down between the tool rest and the abrasive disc while using the grinder, on or about March 12, 2013.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. (a) Shop Tool Cage - Oxygen and fuel gas were stored in the tool cage together on carts. The establishment has two oxygen-acetylene carts for employees to use for cutting and gas welding. The regulators were attached at the cylinder valves and the caps were not in place. Employees were exposed to a fire hazard on or about march 12, 2013.
