29 CFR 1910.23(a)(7): Every temporary floor opening shall have standard railings, or shall be constantly attended by someone. 29 Rig Floor: On or about February 21, 2013, the employer failed to ensure that an opening in the rig floor where a rotary motor was removed for service was covered or equipped with a standard guardrail. Employees were exposed to a fall hazard of approximately 15 ft. to the ground below.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. Rig Floor: On or about February 21, 2013, the employer failed to ensure that adequate fall protection was provided to employee(s) working from the top of the driller's shack. Employee(s) were not protected from a fall from the top of the driller's shack to the rig floor, a distance greater than 4 feet.
