29 CFR 1904.29(b)(6):     The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent:    (a) On or about March 25, 2013 the inspection revealed that for a privacy concern case recorded to the 2010 OSHA 300 log for the McKenzie Bridge Ranger District, the employer did not enter "privacy case" in the space normally used for the employee's name. The OSHA 300 logs were provided by the Supervisors Office, and the privacy concern case number was 204619155.
