29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) Work Floor Near Dock Door No. 119 - A puddle of water approximately 6 feet wide x 10 feet long was observed next to an active forklift operation exposing employees to slipping and sliding hazards, on or about March 28, 2013. No abatement certification or documentation required.

29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: a) Loading Dock, Door #119 - A forklift operator was observed loading/unloading an uncoupled trailer and wheel chocks were not in place exposing the employee to struck-by or caught-in hazards, on or about March 28, 2013. b) Loading Dock, Door #121 - A forklift operator was observed loading/unloading an uncoupled trailer and wheel chocks were not in place exposing the employee to struck-by or caught-in hazards, on or about March 28, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
