29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 2, 2013 on top of the arched section of the roof at the site next to 2994 Turner Hill Road Lithonia, GA 30058;   The employees observed walking on the arched section of the unprotected roof at heights of 23' 8" above ground level to retrieve material were not provided fall protection.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.   On or about April 2, 2013 on top of the parapet section of the roof at the site next to 2994 Turner Hill Road Lithonia, GA 30058;   Employees were observed installing roofing insulation approximately 16' 10" above the lower level and adjacent to a parapet wall that had a height of approximately 8 to 16 inches.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:(a)   On or about April 2, 2013 on top of the parapet section of the roof at the site next to 2994 Turner Hill Road Lithonia, GA 30058;  The employees were not provided a safe means to access the 3' elevated arched area of the roof from the lower level of the roof area while retrieving material.
