29 CFR 1910.303(b)(2):  Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  1) On April 3, 2013, and at prior times, employees were exposed to electrical hazards where power strips were daisy chained which is not in accordance with the equipment's UL listing.    2) On April 3, 2013, and at prior times, employees were exposed to electrical hazards where power strips were plugged into extension cords which is not in accordance with the equipment's UL listing.

29 CFR 1910.334(a)(1):  Flexible electrical cords were fastened with staples or otherwise hung in such a fashion as could damage the outer jacket or insulation:  1) On April 3, 2013, and at prior times, extension cords throughout the lab were hung through holes in metal and fastened with zip ties which could expose employees to electrical hazards.
