29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    a)  Employees were working inside a 3 foot tall valve vault at the bottom of a trench section having a depth of approximately 7.2' without a stairway or ladder provided for safe access to, and egress from, the valve vault.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
