29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  a) Corner of South 12th Street and Rodman Street - Employees drilling holes and setting reinforcement steel in an excavation with vertical walls of approximately eleven (11) feet were exposed to a struck-by hazard as an adequate protective system was not utilized.  On or about April 9, 2013.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a) S 12th St between Lombard St and Rodman St  The portable ladder used to access the excavation did not extend three (3) feet above the upper landing surface.  This condition exposed employees to a fall hazard during access and/or egress.  Observed on or about April 9, 2013.
