29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a) On or 4/12/12, presses B O7 and B O8 guards covering flywheel were damage or not totally enclosed, exposed employees hands  to possible injuries from rotating flywheel. 29 CFR 1910.213(b)(3):  On applications where injury to the operator might result if motors were to restart after power failures, provision shall be made to prevent machines from automatically restarting upon restoration of power.    a) On or about 4/17/13 in the maintenance shop, TS 2 table saw was not equipped with a devise to prevent automatically restarting upon restoration of power after power failure.
