29 CFR 1910.119(h)(3)(ii): The contract employer did not assure that each contract employee is instructed in the known potential fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan  (a) Snavely's Mill - Employees worked at a facility that contains chlorine at a level covered by process safety management.  The contract employer did not ensure that each contract employee had an understanding of the potential fire, explosion or toxic release hazard related to the job and the applicable provisions of the company's emergency action plan, on or about March 16, 2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a)  Snavely's Mill, Cleaning House - Employees worked on a scourer machine. The employer did not insure that each employee working on the removal, place a lock on the energy source, on or about March 14, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
