29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:     a) Facility, 55 South Washington Road, Donora, PA:  On or about April 16, 2013. employees were working in wetter than usual conditions on the 310 line because the expansion tank on the water heater was leaking and the water pump was also leaking which created an excess of water in the water pump room than flowed out under the Banburry machine.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     a) Facility, 55 South Washington Street, PA:  On or about April 16, 2013, a relocatable multi-outlet surge protector was not used in accordance with its UL listing and labeling, exposing employees to shock hazards.  The device was used in a very dusty, manufacturing setting to power a fan and a scale.
