29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about April 17, 2013 at the above addressed site, an employee performing work installing decorative shutters on a walking/working surface with unprotected edges approximately 13 ft. 8 inches above a lower level was not protected from falling to that lower level.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder:  On or about April 17, 2013 at the above addressed site, an employee gaining access to and from a walking/working surface for the installation of decorative shutters approximately 13 ft. 8 inches above a lower level using an aluminum extension ladder set up in an active driveway did not have that ladder secured to prevent accidental displacement.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  On or about April 17, 2013 at the above addressed site, an employee gaining access to and from a  walking/working surface approximately 13 ft. 8 inches above a lower level using an aluminum extension ladder was carrying a box of plastic anchors which could cause him to lose balance and fall.
