29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components shall conform to the criteria in � 1926.502:    A) East side of building 680, on or about April 15, 2013, an employee was using a roof anchor bracket as part of a fall arrest system. The roof bracket was not installed under the supervision of a qualified person. The bracket was attached to the side of the building and only had two screws securing it, exposing employees to a fall hazard of approximately 22 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee.     Roof of Building 680, on or about April 15, 2013, an exposed employee acted in a manner that indicated the employee had not been retained the requisite understanding  and skill in the use of the fall protection systems and the employee had not been retrained.
