29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about April 22, 2013, at the above address jobsite:  The employer did not develop, implement, and maintain a written hazard communication program to provide labels and other forms of warning, safety data sheets, employee information and training for employees who could be exposed to potential chemical hazards including but not limited to portland cement.   PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about April 22, 2013, at the above address jobsite:  An employee performing masonry work applying stucco from the basket of an aerial lift approximately 35 feet above a lower level was not wearing a harness with a lanyard attached to the boom or basket of that lift.
