29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a) Super 8 Motel, 8040 B Perry Highway in Erie, PA.  On or about April 26, 2013, and times prior thereto, the employer did not develop, document and implement a lockout/tagout program consisting of written procedures, training of authorized employees, training of affected employees, and periodic inspections of the program procedures.

On or about April 26, 2013, and times prior thereto, employees working on electrical equipment such as, but not limited to electrical outlets, lighting and other fixtures, did not receive workplace specific training on the electrical lockout procedures associated with and noted in the 29 CFR 1910.333(b).
