29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a.  615 Patrick Industrial Drive, Winder, GA (Shop Area) - The employees were exposed to sudden energization or start-up of multi-energy source machines.  The employer did not develop and implement a lockout/tagout program with machine specific written procedures for locking out/tagging out the multi-energy machines to safeguard employees performing services and/or maintenance on machinery or equipment, on or about 05/01/13.       a.  Omada bandsaws - (220-240V, hydraulic)      b.  Press Brake - (440V, hydraulic)      c.  Metal Shear - (480V, hydraulic)      d.  Burn Table/Plasma Table - (480V, 120 psi, thermal gas)      e. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a.  615 Patrick Industrial Drive, Winder, GA (Shop Area) - Employees were exposed to chemical and fume hazards.  The employer did not develop and implement a written hazard communication program where employees were exposed to hazardous chemicals on the jobsite. The hazardous material includes, but is not limited to, compressed gas, oil, lacquer thinner, and stainless and carbon steel on which employees are performing grinding, cutting, and welding activities which creates metal fumes due to disturbing the molecules of the steel, on or about 05/01/13.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a.  615 Patrick Industrial Drive, Winder, GA (Shop Area) - Employees were exposed to chemical and fume hazards. Employees were not provided with training and information on the types of hazardous fumes and chemicals associated with the fabrication of metal materials.  Employees work with chemical products such as, but not limited to, compressed gas, oil, lacquer thinner, stainless and carbon steel on which employees are performing grinding, cutting and welding activities.  Employees were also not informed how to be familiar with the Safety Data Sheets for the chemicals and metals, on or about 05/01/13.
