(a) On or about May 1, 2013, lock out procedures were not used for the control of hazardous energy while entering the grain storage bins, exposing the employee to the hazard of being caught-in the rotating parts of the main auger.    (b) On or about May 1, 2013, lock out procedures were not used for the control of hazardous energy while entering the grain storage bins, exposing the employee to the hazard of being caught-in the rotating parts of the sweep auger.
