29 CFR 1926.403(b)(1)(ii): Safety of equipment was not determined on the basis of mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:    a) 3200 Chestnut St, Philadelphia, PA  The energized connection/splice point (120 volts) between the temporary wiring and the temporary electrical outlets were not sufficiently guarded with durability against accidental contact in several locations throughout the job site, exposing the employees to an electrocution/shock hazard.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:    a) 3200 Chestnut St, Philadelphia, PA  Live parts of electrical equipment was not guarded against accidental contact. The energized (208/110 volts) circuit breaker panel was missing circuit breakers or blanks, exposing the energized bus bar which exposed the employees to an electrocution hazard.This violation was observed on or about May 19, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas and passageways:  a)3200 Chestnut St, Philadelphia, PA  Debris was not cleared from work areas and passageways, exposing the employees to trip and fall hazards also impeding the employees during an emergency egress. This violation was observed on or about April 22, 2013.   Abatement certification required within 10 days after abatement date.
