29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee   a) Weatherproofing installation, VA Medical Center - On or about 5/7/13, employees were exposed to fall hazards when a base frame for a platform was used as an anchorage point and was not capable of supporting 5000 lbs.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   a) Weatherproofing installation, VA Memphis, On or about 5/7/13, employees were exposed to fall hazards when the lifeline system did not limit falls to 6 feet.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
