29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes, or loose boards.    WORKSHOP:  The areas of the inclined and uneven floor surface of the Bobcat Den walk-in refrigerator had loosed pieces of metal and screws, creating trip and fall hazards to the employees.      On or about April 24, 2013, an employee tripped over a piece of loosed rug on the floor while walking out of the above walk-in refrigerator.      ABATED DURING THE INSPECTION.
