29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, shall be guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings.    (a) Truck Shop, Service Pit for Yard Mule at Bay #4 - On or about April 30, 2013, employees were exposed to falling 54 inches down into the 3 foot wide by 4 1/2 foot long uncovered/open end of the service pit at Bay #4.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  (a) Truck Maintenance Shop, Wash Room in old Truck Wash Bay - On or about May 1, 2013, employees were exposed to electrical shock hazards due to the absence three circuit-breakers, or three replacement knock-out plates, over the openings for spaces #2, #13 & #19 inside the 200 amp, 120-240 VAC, 3 phase Square D disconnect panel which controls the diesel pumps at the Fuel Island.

29 CFR 1910.1200(f)(6): Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: -The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, -Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  (a) North of Truck Maintenance Shop, at the Diesel Storage Tank Area - On or about May 1, 2013, employees were exposed to health hazards due to the absence of any labeling on the 60 foot long, 35,000 gallon capacity Diesel Tank #3.
