29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them: On or about May 14, 2013, employees perform maintenance and servicing on equipment such as but not limited to boilers; dryers; Pre-expansion, water, vacuum, and bead systems; air compressor; cooling tower; and presses. Employees were exposed to struck-by hazards when the machine specific lockout/tagout procedures did not include specific steps for removal of the locks and/or other energy isolating devices.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about May 14, 2013, employees were exposed to struck-by hazards when the annual lockout/tagout review was approximately two weeks late and was used to audit employees' skills and not to identify deficiencies in the procedures such as but not limited to specific steps for the removal of devices.
