29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). (a) Jobsite - On or about May 13, 2013 an employee was operating a Toyota forklift without being trained and evaluated on how to operate forklift safely.

(a) Jobsite - On or about May 13, 2013, three forklifts were found to be in need of servicing: (i) Taylor T180 forklfit had an inoperable horn and a damaged rear right tire (ii) Taylor T200 forklift had an inoperable horn and lights and a damaged front lift tire (iii) Toyota forklift had an inoperable right headlight

Safety of equipment shall be determined using the following considerations: (a) Framing Ramp - On or about May 13, 2013 ground prongs were broken off and lodge inside two electrical outlets causing an electric shock hazard to employees using the outlets. (i) outlet on the pole underneath the framing shed (ii) outlet on the outside of the equipment storage shed
