29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.    a) In the Blender Area / UD, on or about and at times preceding to May 15th, 2013, an emergency eyewash/body flush station was not immediately accessible in area where Sulfamic Acid was used in the blending operations.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used:   a) Throughout the work facility, on or about and at times preceding to May 15, 2013, employees required to utilize 3M 8511 filtering facepiece respirators were only fit tested to wear full face respirators.
