29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  On or about May 22, 2013, the guard covering the top feed roller for the Emba 240 left a 3/8 inch to 1/2 inch opening over the C Flute corrugated board stock.  Employees fingers were able to pass under the guard on the stock.  Employees were exposed to the in-running nip points of the feed rollers.      (b)  On or about May 17, 2013, the rotating top feed roller for the Hooper Printer/Slotter was open and exposed to employees.  Employees were exposed to the rotating in-running nip point of the roller and the stationary guard located just above the feed table.      (c)  On or about May 31, 2013, the rotating top feed roller for the S & S 2C Printer/Slotter was open and exposed to employees.  Employees were exposed to the rotating in-running nip point of the roller and the stationary guard located just above the feed table.    No abatement certification or documentation is required for this item.
