29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.    This violation was observed on May 28, 2013 at a job site located at Track# 159, Lot# 9, Block# 1, Yigo GU, where workers were exposed to impalement hazards while walking and working around the two-storey concrete building project.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     This violation was discovered on 5/28/2013 at a job site located at Track# 159, Lot# 9, Block# 1, Yigo, GU 96929 where an employee performed work on the second level of concrete building and was exposed to a fall hazard of 8 feet 6 inches to the lower level.
