29 CFR 1926.34(a): Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied:    a) Oxford Mills, West Building - Employees were not provided with free and unobstructed egress from the West Building at the concrete stairs in the Hope Street alleyway.  The blasting hoses ran across the the stairway landing, exposing employees to injuries.  Observed on or about 4/5/13.       Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.62(h)(1): "All surfaces shall be maintained as free as practicable of accumulations of lead".    a) Oxford Mills, East and West Buildings - exposing subcontractors and their employees to lead.  Wipe samples of the floor and other surfaces in the East and West Buildings were taken on 4/5/13, 4/8/13, 5/13/13, and 7/10/13 and contained between 317 to 3829 micrograms of lead per 100 square centimeters.       NOTE:  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The floor of the East Building was approximately 34 inches above the courtyard and the employer did not provide a stairway, exposing employees to a fall hazard.  Observed on or about 7/10/13. 29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stairrail system along each unprotected side or edge      a) Oxford Mills, West Building - The concrete stairway leading into the West Building from Hope Street, was constructed with five risers and rose approximately 46 inches.  The stairway did not have a stairrail system along the unprotected side, exposing employees to fall hazards.  Observed on or about 4/5/13.
