29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails. At the time of the inspection, the fixed ladder to access the roof did not have side rails that extended above the level of the roof. Abatement certification and abatement documentation is required for this violation.
