29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)  (a)  At the site - Employees were observed accessing the roof via a single unguarded plank exposing them to a 10 foot fall.  American Eagle Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.451(b)(2), which was contained in OSHA inspection number 315977017, citation number 01, item number 001 and was affirmed as a final order on 10/11/2011, with respect to a workplace located in at 1838 London Crest Lakes Sherwood Ocoee, FL 32818.  American Eagle Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.451(b)(2), which was contained in OSHA inspection number 316106277, citation number 01, item number 001 and was affirmed as a final order on 11/21/2011, with respect to a workplace located in at 1838 London Crest Lakes Sherwood Ocoee, FL 32818.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a)  At the site - Employees were observed climbing the components of the tubular welded frame scaffold system to gain access to the work platform.  American Eagle Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.451(e)(1), which was contained in OSHA inspection number 315977017, citation number 01, item number 002 and was affirmed as a final order on 10/11/2011, with respect to a workplace located in at 1838 London Crest Lakes Sherwood Ocoee, FL 32818.
