29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) 5 Forest Park Ave Springfield, MA: On or about June 5, 2013, employees working on this residential roof were not protected from falling more than 25 Ft to the ground below which is not in accordance with this subpart:   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  (a) 5 Forest Park Ave Springfield, MA: On or about June 5, 2013, employees were observed using an extension ladder, to access the 3rd story roof, that did not extend 3 FT past the roof surface as required by this subpart.
