29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) Big Bear Construction Co. @ 4903 Boca Del Rio, Fort Collins, 80524: On and before June 4th, 2013, the exposing employer did not ensure employees wore protective helmets when exposed to overhead hazards such as, but not limited to, falling tools and materials. The condition exposed employees to head injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a) Big Bear Construction Co @ 4903 Boca Del Rio, Fort Collins, CO 80524: On and before June 4th, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in framing operations while walking and working on the second level enaged in framing operations on a single-family home under construction. Employees were not utilizing a fall protective system. Employees were exposed to fall hazard of approximately 10'.
