29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable:     The employer is failing to protect employees from potential slip, trip and fall hazards associated with wet walking/working surfaces on the job site.  This was most recently evident during the inspection of the facility located at 16-240 Mikahala St., Shipman Business Park, Keaau, HI 96749, where the employer has employees working in the outdoor refinery plant with new smooth concrete floors and annual rainfall of over 145 inches creating slippery walking conditions and potential struck by hazards all along the edges of the walkways.

This was most recently evident during the inspection of the trailer loading/unloading area of the facility located at 16-240 Mikahala St., Shipman Business Park, Keaau, HI 96749, where the employer has employees accessing the top of tanker trailers by a portable ladder, so they can take samples from the tanks using a dipping wand without having any means of protection from falling to the ground over 13 feet below.

This was most recently evident during the inspection of the facility located at 16-240 Mikahala St., Shipman Business Park, Keaau, HI 96749, where the employer has employees climbing on top of ISO tanker trailers to check volume levels for the waste water which contains an unknown amount of methanol without having conducted air quality testing to determine if a type of forced air respirator is required.
