This was most recently evident during the inspection of the facility located at 16-240 Mikahala St., Shipman Business Park, Keaau, HI 96749, where the employer has employees working in the outdoor refinery plant with new smooth concrete floors and annual rainfall of over 145 inches creating slippery walking conditions and potential struck by hazards all along the edges of the walkways.
