29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:     (a) Jobsite, 217 Main Street, East Conemaugh, PA:  On or about 06/12/13 the 24 ft. platform erected on the ladder jack scaffold extended on each side of the ladder supports approximately 5 feet and were not secured in place to prevent movement.  No guardrails were in place to prevent the employees from stepping onto the cantilevered end exposing employees to a potential fall hazard of approximately 16 feet to the ground below.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:     (a) Jobsite, 217 Main Street, East Conemaugh, PA:  On or about 06/07/13 employees working from the ladder jack scaffold installing soffit and fascia, approximately 16 feet above the ground, were not protected from falling by the use of a personal fall arrest system.

Employees were working at heights up to approximately 20 feet above the ground and not utilizing any fall protection systems.  The employees had not been trained in the procedures to be followed to minimize exposure to falls.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   (a) Jobsite, 217 Main Street, East Conemaugh, PA:  On or about 06/12/13 the employer did not train employees who work from scaffolding to recognize the hazards associated with the type of scaffold being used and what procedures or methods to use to minimize those hazards.  Employees were working on a ladder jack scaffold with platform installation deficiencies and no fall protection.
