(b)  In the sewing shop:  On June 13, 2013 and at times prior to; the employer failed to guard the pulley on the LU 563 industrial sewing machine exposing employees to caught-in hazards during sewing operations.  (c) In the sewing shop: On June 13, 2013 and at times prior to; the employer failed to guard the pulley on the TACSEW Model 1563 industrial sewing machine exposing employees to caught-in hazards during sewing operations.
