29 CFR 1926.404(b)(1)(iiii)(C): Where an an assured equipment grounding program was being utilized, the employer did not ensure that each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, was visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: a) Main floor mechanical room: On June 14, 2013, and times prior thereto, employees were exposed to an energized temporary electrical cord that had the conductors pulled out of the strain relief device on the cord cap and taped with electrical tape to hold it together. Note: Abatement certification is Not required for this item.
