29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party:    a) Drexel University (Law Building): An employee (custodian) maintained an unlabeled container of either Multi Surface cleaner containing Phosphoric Acid, Ethoxylated Linear Alcohol, Hydrogen Peroxide, Propylene Glycol N-Propyl Ether and Alkylbenzene Sulphonic Acid in the first floor janitorial closet. The employer did not ensure that the container was labeled, tagged or marked with the product they contained. On or about June 18, 2013.     b) Drexel University (Law Building): An employee (custodian) maintained an unlabeled spray bottle of Glance Glass Cleaner containing Sodium Lauryl Sulfate and Ethyl Alcohol in the first floor janitorial closet. The employer did not ensure that the container was labeled, tagged or marked with the product they contained. On or about June 18, 2013.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) Drexel University: An employee, (Custodian) while dusting, was provided with and wore Sperian Light Duty CP1000VP Masks. The employer did not provide the employee with Appendix D of the Respiratory Protection Standard. On or about June 18, 2013.
