WORKSITE: The employer did not ensure through specfic instruction and/or visual observation that anybody was free from, and/or not in close proximity of the load prior to hoisting.    On or about 06/21/2013, a traffic signal pole (pole) was hoisted by the Altec Digger-Derrick, Model D945, prior to an employee completely unscrewing the nuts of the base plate of, and completely free from the pole/load. The employee's fingers were caught in between the nuts and plate, causing serious injuries to the fingers.

29 CFR 1910.180(h)(3)(vi): The operator should avoid carrying loads over people.    WORKSITE: The employer did not ensure that workers were away from the load before hoisting the load.    On or about 6/21/2013 a worker was unscrewing the nuts off a threaded bolt in order for the traffic pole to be removed when the operator of the Altec, model D945 Digger-Derrick, Serial Number: 0199BE2588 hoisted the pole, catching the workers left 4th and 5th fingers between the threaded bolt and nut. The worker sustained serious injuries to his fingers.
