Cement blocks were installed on the back side of the forklift to increase the lifting capacity. This exposed the operator to a crushing hazard from overloading of the forklift.    (b) Towmotor Forklift - Serial No. 3555-61: The Towmotor forklift was modified without manufacturer's approval. There were torch cut holes to allow ball hitches to be attached to the forks.
