29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a)  On or about 06/21/2013 - Facility, procedures were not developed and documented for employees engaged in the servicing and maintenance of equipment such as, but not limited to, Komatsu 300 ton presses, Komatsu 200 ton press, Komatsu 80 ton press, Komatsu 60 ton presses, Cincinnati shear, Vibra-Deburring machines, Turrent 30 ton presses, and brake presses.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure:    (a)  On or about 06/21/2013 - Facility, employees trained as first responders had not been offered the hepatitis B vaccination.
