29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    a. On or about June 28, 2013, at the above located workplace, employees were required to operate a powered industrial vehicle (Clark TMG 25) during material handling operations. The employer did not ensure that each powered industrial truck operator completed the required training and evaluation specified in paragraph (l).
