On 7/15/13, employees were working on a ladderjack scaffold platform where the ladder used to support the ladder jack scaffold was a Werner Class 1 (275# capacity) extension ladder that had been separated into two pieces, with the fly section used to support one end.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On 7/15/13, employees were re-roofing a two story house without acceptable means of fall protection to prevent falling from the roof in that:    a) One employee was not wearing a harness.    b) One employee was wearing a harness that was not connected to a lifeline.    c) Two employees had lifelines that were connected to one anchor.    d) One employee had his lifeline tied to the anchor.    e) Two employees had lanyards connected to lifelines by knots.    f) Two nail down roof anchors were improperly installed without adequate fasteners.
