29 CFR 1926.201(a):  Flaggers. Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers shall conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), which are incorporated by reference in ��1926.200(g)(2).  a.  At the site located at Schwartz Place, Lancaster, Ohio, an employee was holding a traffic control sign in the road without wearing high-visibility clothing exposing the employee to a struck-by hazard.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Rockford Corporation in Lancaster, Ohio:  a.  On the 2013 OSHA 300 Log the employer did not provide the information required in Column D, the date of injury or onset of illness for case number 1.  b.  On the 2013 OSHA 300 Log the employer did not provide the information required in Column D, the date of injury or onset of illness for case number 2.  c.  On the 2013 OSHA 300 Log the employer did not provide the information required in Column D, the date of injury or onset of illness for case number 3.
