29 CFR 1926.416(a)(1):     Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  1600 Adams Street, In the Sludge-Blending Room, Hoboken, NJ.     Employees were exposed to electric shock as the 480V/277V panel had its deadfront off and wires from the disconnect switch to this panel had been energized to power the overhead lights.                                                             Violation observed on or about 07/19/2013.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  1600 Adams Street, Hoboken, NJ in the Sludge Blending Room of the Sludge Blending and Storage Facility.    Employee was exposed to 7 foot, 10 inch falls to the concrete floor below from the pipe conduit he was working from as no methods of fall protection were in use.                                                Violation observed on or abouit 07/19/2013.
