29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) At the worksite, an energy control program was not established for the maintenance and repair for equipment such as, but not limited to, the MRK-55 mill. This condition exposed employees to electrical, struck-by, caught-in, and struck by hazards.

29 CFR 1910.303(b)(1)(i): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment had listing or labeling identifying the purpose: a) Inside Building 1, a non-industrial power strip was used to power tools and equipment near the workbench. b) Inside Building 4, a non-industrial power strip was used to power the Micro - Hite 900 precision tool c) Inside Building 4, a non-industrial power strip was used to power the Harding mill and additional equipment.
