29 CFR 1917.151(g)(1): Safety guards for abrasive wheels were not adjustable or did not have an adjustable tongue or piece at the top of the opening where the operator must stand in front of the safety guard opening.     a.  Terminal, High Lift Maintenance and Chassis Maintenance - Noted on or about 8/2/13 and at times prior thereto, it was observed that a Milwaukee bench grinder had a measured 1/2" gap between the tongue guard and the abrasive wheel and a U.S. Electrical floor mounted, double wheeled grinder had no tongue guards on either side.

29 CFR 1917.151(g)(4): Work rests on fixed grinding machines were not adjusted closely to the wheel with a maximum opening of 1/8-inch (3.18 mm) and securely clamped.   a.  Terminal, High Lift Maintenance - Noted on or about 8/2/13 and at times prior thereto, it was observed that the workrests of a double wheeled Milwaukee bench grinder had a measured 1/2" gap between the work rests and the abrasive wheels on both sides.

29 CFR 1918.105(b)(1): The employer did not provide personal flotation devices (PFDs) for each employee engaged in work in which the employee might have fallen into the water.   a.  Pier 39, Cargo/Barge Operations - Noted on or about 8/2/13 and at times prior thereto, an employee was observed from the terminal yard climbing over a barge railing that was actively being loaded thereby exposing him to to a fall distance of approximately 10 feet into the water below which is 35 feet deep.   Other employees were observed working nearby the employee on the barge deck and the nearest throw ring is located on the barge approximately 150 feet away.
