29 CFR 1910.303(b)(1)(i): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment had listing or labeling identifying the purpose for which the equipment was designed:   At this work place employees performed electrical operations at temporary electrical panels and disconnected energized 120VAC 3 Phase branch circuits attached to the panel board by in adequate strain relief devices.  Employees were exposed to electrical hazards at the following panels on the vessel's top deck:        a)    Temporary Electrical Panel #20      b)    Temporary Electrical Panel #28      c)    Temporary Electrical Panel #31

29 CFR 1910.305(a)(2)(viii): Suitable disconnecting switches or plug connectors were not installed to permit the disconnection of all ungrounded conductors of each temporary circuit. Multiwire branch circuits were not provided with a means to disconnect simultaneously all ungrounded conductors at the power outlet or panelboard where the branch circuit originated:    At this work place employees performed electrical operations at temporary electrical panels and disconnected energized 120VAC 3 Phase branch circuits labeled white, black and red from the panel board individually.  Employees were exposed to electrical hazards at the following panels on the vessel's top deck :       a) Temporary Electrical Panel #20      b) Temporary Electrical Panel #28      c) Temporary Electrical Panel #31
