29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On August 6, 2013 during the tear off and installation of roofing material, the employer did not ensure employees working on a roof 16 feet from the ground with an approximately 5/12 pitch were protected by a guardrail system, safety net system, and/or personal fall arrest system.     BENNY TROYER ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 691958, CITATION NUMBER 02, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 15, 2012 WITH RESPECT TO A WORK PLACE LOCATED AT 1066 EASTLAND AVENUE, AKRON, OHIO.
